A just culture approach, using human factors and systems thinking, helps community organisations, such as disability services providers, build a culture where everyone feels safe to speak up and share information, processes and procedures are improved based on a shared understanding and meaningful engagement of staff and clients, and the organisation responds with a focus on learning and proactive and continuous improvement rather than blame and punishment.
