This project aimed to investigate and document experiences of gender-based violence occurring in adult mental health inpatient units and perpetrated against those identifying as women, in order to inform how policy and practice can be improved to make these environments safe for women.

This strategy collection describes how the NHS will continuously aim to improve patient safety, building on the foundations of a safer culture and safer systems.

Australia could save $1.5 billion a year on health spending by improving the safety of patient care in hospitals. Safer hospital care doesn’t just reduce harm to patients, it also saves money for taxpayers.

Failure to follow up abnormal test results is an important patient safety issue and requires urgent attention.

This report argues that Australia needs to reform the way we collect and use information about patient safety, to reduce the risk of more scandals and tragedies in our hospitals.
