This review examined the evidence of risks to patient safety in primary healthcare and the effectiveness of interventions to minimise risks.
It found an increased risk of harm was associated with patient factors such as age or co-morbidities, and reduced risk was associated with being registered with a practice for a longer time and having longer clinical consultations.
Using a range of methods is optimal in investigating incidents.
