Recognising diverse expertise makes for better policy, the Australia Cares project was founded to enable community-led policy development shaped by lived experience, and community and academic expertise. Drawing together the findings of 18 months of collaboration, deliberation and experimentation, this report marks the conclusion of the first phase of the project.

This study provides empirical data on the experiences of isolated older Australians living in New South Wales and the effects of social and geographical isolation on these older people. It also provides empirical data on how service providers across New South Wales identify isolated older people and facilitate their access to supports and services.

This report is the culmination of a two-year collaborative project between RMIT University's Innovation Catalyst and The Constellation Project, which focused on reviewing, further developing, and sharing Constellation’s model of lived experience inclusion.
