This report documents the development of a systematic, rigorous co-design process to develop an age-friendly approach to care for older people.
The Indigo 4Ms Framework combines the great strengths of the Institute for Healthcare Improvement’s (IHI) 4Ms Framework and the World Health Organization’s (WHO) Integrated Care for Older People (ICOPE) guidelines to deliver better care for all older people living in rural and regional Victoria.
The Indigo 4Ms Framework brings together the essential elements of the best care for older people — what matters, medications, mobility and mental wellbeing. A set of key actions for each of four core elements has been developed. These four interdependent elements must be considered in every interaction with every older person, irrespective of setting, health or social care worker, or the level of functional ability of the person.
